It is vital to keep everyone safe, and as such we have put in place clinic processes and procedures. These are some steps that we’ve taken to reduce transmission of Covid-19.

One patient in the clinic at a time, with more time between patients to allow for cleaning.
Thorough cleaning of all surfaces between each treatment.
Contactless payments where possible. I can send you a link so that payments can be made online

Covid-19 Consent Form

At Tam Chiropractic Clinic we are doing all that we reasonably can to minimise risk whilst conducting face-to-face appointments, however, we cannot totally eliminate risk of COVID-19 transmission. Patients should not attend if they think they are beginning to exhibit COVID-19 symptoms, please contact us to reschedule your appointment.

I am not experiencing any COVID-19 symptoms such as fever, cough or loss of sense of smell at the moment. If these symptoms do arise in the future I will notify my chiropractor and will not attend the clinic.

I understand that there may be a risk of transmission of COVID-19 as a result of attending the clinic.

I have not been in close contact with a Covid-19 patient in the last 14 days.

I have read and understand the COVID-19 information page above, and am satisfied the necassary precautions have been taken for my visit. I have had the chance to ask all the questions I wish to at this time.

I consent to receive care at Tam Chiropractic Clinic.
